By entering my registration, I agree that substitutions to my registration may be made at any time, for any reason by calling 800-531-1026 or 603-357-8101, 8am - 5pm EST. I understand that my registration fee will be refunded, minus a $150 processing fee, if requested on or before March 24, 2013. Refund requests after close of business on March 24, 2013 will be provided at 50% of your total registration fees. Non-attendance (without appropriate cancellation as described above) will incur the full event fee.

With my registration I hereby give full permission to Kennedy Information, LLC (parent company of G2 Intelligence) to use my photographic or video image(s) (“Picture(s)” )in whole or in part and in any media whatsoever, obtained as part of my attendance at MDx NEXT. I release and hold harmless Kennedy Information, LLC from any liability by virtue of any use my Picture(s) and/or contact information, including without limitation any claims for libel or invasion of privacy. I certify that I am at least 18 years of age, that I have the authority to grant the permission contained herein, and that this release is given voluntarily and without expectation of compensation in any form now or in the future.
